Medical Records Manager processes and organizes medical records in
a timely manner and in compliance with regulatory and accrediting
agencies, as well as state and federal laws. Ensures completeness
of the record by auditing charts and completing a medical records
checklist. Handles the storage and retention of charts, responds to
requests for medical record, copies and maintains medical record
tracking log. Provides support to the facility by performing
specific or various business office functions as assigned. These
functions are to include (but not limited to): Medical Records,
Credentialing, Admissions/Intake, Accounts Payable, Billing,
Collections, Insurance Verification, Transaction, Posting, Clinical
Logs and other duties as assigned. Functions within the Center's
policies and procedures, support SCA Values, SCA Vision and SCA
Mission. Key Responsibilities:
- Chart Completion
- Ensures all documents per audit sheet are in the chart.
- Operative Report:
- Receives operative notes from transcription and review for
completeness and accuracy. i.e.: Is patient name spelled
correctly?
- Logs op notes. Request outstanding op notes from physician,
following facility protocol
- Sends operative notes to surgeons, PA, cc's...
- Medical Record Request:
- Log all requests from outside sources. (patient, attorney,
insurance co.)
- Ensures requests have appropriate patient
signature/approval.
- Reviews verbiage for prohibitive items: (HIV, alcohol
treatment...)
- Requests records from storage, as needed
- Retrieving records:
- Reviews schedule for dates of service (2 weeks ahead) and look
for repeat patients
- Locates, signs out, and delivers medical records requested by
hospital departments
- Filing:
- Files or scans completed charts daily
- Maintains efficient and orderly filing system for both
completed and incomplete records
- Periodically purges outdated records and prepare for
storage
- Maintains "Out Cards" when charts are pulled
- Record Storage and Retention:
- Ensures medical records are retained per state guidelines, at a
minimum of 10 years
- Ensures patients who are minors at time of surgery have charts
retained longer
- Ensures access to outside record storage is restricted to
facility management and key personnel
- Before sending charts to storage, logs each chart and compares
to list of patients from PAS. Notes and researches missing
charts
- Maintains log of charts received and returned to storage
- Versatile and willing to coordinate with and participate in,
when necessary, all other aspects of the Business Office
- Demonstrates ability to handle multiple tasks
- Demonstrates ability to enter data into computer, files large
amounts of paper, and uses office equipment
- Responsible for oversight of non-medical personnel, including
orientation
- Acts as liaison with insurance companies Qualifications
- High school diploma or equivalent required
- Associate's Degree or equivalent from a two-year College or
technical school preferred
- Three - five years' experience in a medical office, hospital,
outpatient surgery center or related field preferred
- Computer experience, Excel, Word, Medical Billing Software and
Applications
- Working knowledge of Medical Terminology
