Physical Therapist PT Responsibilities:- Provide personalized one-to-one care to clients based on individualized Plan of Care- Monitor and report changes in clients' conditions and needs to the Director of Nursing- Keep clients' families informed about care plans, health changes, and any concerns
Physical Therapist PT Requirements:- Current unrestricted PT license in the state where care is being provided- Certified BLS CPR(AHA In-Person) and negative TB skin test or chest x-ray- Minimum of one year documented experience in a home health agency, primary care clinic, or health facility preferred- Valid driver's license and automobile insurance- Ability to perform physical tasks such as standing, walking, bending, lifting up to 50 pounds, and assisting clients with mobility
