Accountabilities / ResponsibilitiesThe Medical Records Manager
processes and organizes medical records in a timely manner and in
compliance with regulatory and accrediting agencies, as well as
state and federal laws. Ensures completeness of the record by
auditing charts and completing a medical records checklist. Handles
the storage and retention of charts, responds to requests for
medical record, copies and maintains medical record tracking log.
Provides support to the facility by performing specific or various
business office functions as assigned. These functions are to
include (but not limited to): Medical Records, Credentialing,
Admissions/Intake, Accounts Payable, Billing, Collections,
Insurance Verification, Transaction, Posting, Clinical Logs and
other duties as assigned. Functions within the Center's policies
and procedures, support SCA Values, SCA Vision and SCA Mission. Key
Responsibilities:Chart CompletionEnsures all documents per audit
sheet are in the chart.Operative Report:Receives operative notes
from transcription and review for completeness and accuracy. i.e.:
Is patient name spelled correctly?Logs op notes. Request
outstanding op notes from physician, following facility
protocolSends operative notes to surgeons, PA, cc's...Medical
Record Request:Log all requests from outside sources. (patient,
attorney, insurance co.)Ensures requests have appropriate patient
signature/approval.Reviews verbiage for prohibitive items: (HIV,
alcohol treatment...)Requests records from storage, as
neededRetrieving records:Reviews schedule for dates of service (2
weeks ahead) and look for repeat patientsLocates, signs out, and
delivers medical records requested by hospital
departmentsFiling:Files or scans completed charts dailyMaintains
efficient and orderly filing system for both completed and
incomplete recordsPeriodically purges outdated records and prepare
for storageMaintains "Out Cards" when charts are pulledRecord
Storage and Retention:Ensures medical records are retained per
state guidelines, at a minimum of 10 yearsEnsures patients who are
minors at time of surgery have charts retained longerEnsures access
to outside record storage is restricted to facility management and
key personnelBefore sending charts to storage, logs each chart and
compares to list of patients from PAS. Notes and researches missing
chartsMaintains log of charts received and returned to
storageVersatile and willing to coordinate with and participate in,
when necessary, all other aspects of the Business
OfficeDemonstrates ability to handle multiple tasksDemonstrates
ability to enter data into computer, files large amounts of paper,
and uses office equipmentResponsible for oversight of non-medical
personnel, including orientationActs as liaison with insurance
companies QualificationsHigh school diploma or equivalent
requiredAssociate's Degree or equivalent from a two-year College or
technical school preferredThree - five years' experience in a
medical office, hospital, outpatient surgery center or related
field preferredComputer experience, Excel, Word, Medical Billing
Software and ApplicationsWorking knowledge of Medical Terminology
